$2500 - $5500 Deductible, First $500 Preventive Services Covered in Full, 0% In / 25% Out-of-Network Office Visits & Coinsurance
Clear Saver
$1000 - $5000 Deductible, $30 Copay Office Visits and Preventive Services, 30% In / 50% Out-of-Network Coinsurance
Uniform Plan
$1000 Deductible, 50% In-Network Office Visits & Coinsurance, Urgent & Emergency Care Only Out-of-Network
